Connects people with quality in-home and community-based health care; provides information, direct access to qualified care providers and many comprehensive

services to help people come home from the hospital sooner or live independently

at home longer.  Services are funded by the Ministry of Health and Long-Term Care

and can include: A range of in-home health care, nursing clinics, related social

services and supplies and equipment; Coordinate the application for admission

to Long-Term Care Homes; offer assessment for referral to specialized programs

geared to specific health needs and provides information about a wide range of

services available in the community.

Geriatric Assessment & Intervention Network (GAIN)


 GAIN teams provide specialized geriatric care to support frail seniors living at home

or in retirement residences, who have multiple complex medical and social problems including: cognitive impairment, responsive behaviours, decreased function, falls or

risk of falls, impaired mobility, elder abuse, incontinence and/or multiple medications.

KPP Attendant Outreach Services provides personal support and homemaking

supports that are essential to support the activities of daily living for adults with

Shelter an accommodation for homeless adult males over the age of 18 years.

Shelter consists of 40 shelter beds for and Open Table community dining program.

Dedicated staff person to assist with transition to successful independent living.

A local community centre that is open to the public year round. It’s a place where

those in need can come to find shelter, food, rest, support and most importantly,

a sense of belonging within our community. Free space for low income individuals;

drop-in centre that provides lunch meals daily.

Safe Beds provide a safe and supportive environment in a residential home

setting for individuals within Peterborough, Halliburton, Kawartha Lakes, and Northumberland Counties. A short-term stay is available to all genders 16 years

of age and older. Safe Beds are a non-medical facility, abstinence based, voluntary,

and of no cost to the individual.
